The virtual care community

GRTHealth provides modular remote monitoring solutions to maximize the ability of seniors, living with frailties, to live safely and cost effectively, in a community setting, as compared to traditional approaches that rely on institutionalization and the need to employ high-cost and scarce health human resources. Through a scalable and hardware agnostic software platform (GRTInsight®), GRTHealth solutions integrate medical and non-medical monitoring devices that have been designed to maximize the abilities of ‘at-risk’ seniors to live independently in the community, and to minimize barriers that may prevent discharges from acute care hospitals. The integrated solutions have also been designed to lengthen community stays for seniors who may be traditionally admitted to a long-term care facility, as well as to minimize unnecessary visits to emergency departments.
